SKY staff will conduct a complimentary pre-admission assessment at the location of the patient, whether it be in a neighboring hospital, or in their home. The preadmission assessment helps to determine how treatment at SKY can help. Ability to participate in the treatment process, and the extent of impairment to daily living skills and functioning are evaluated. The Medical Director reviews each preadmission assessment and gives admission recommendations. The Director of Admissions then facilitates the admission.
If admission to SKY is not recommended following the pre-admission assessment, it may be suggested the patient pursue outpatient therapy, home health care, adult day care, admission to a skilled nursing facility or long-term care facility, among other options. At any point in time patients may request a new pre-admission assessment by calling the admissions office.
Services for individuals who are adolescents and older are offered regardless of race, color, creed, national origin, religion, sex, disability, veteran status, ancestry, marital status, or citizenship. The facility and its programs are fully accessible to patients, guests and staff with a wide variety of disabilities.
